VERIFICATION — claim / clarify / decide

Queues are cursor-bounded. Cases require claim leases with fencing. Clarification freezes approve/reject until resubmit. Decisions need MFA + active lease; approval emits activation work for Prompt 12. Unassigned verifiers cannot open or stream case photos.
